Endocrine Society Guidelines for Pump and CGM Use in Adults


In September 2016 the Journal of Clinical Endocrinology & Metabolism published guidelines from the Endocrine Society for the use of Continuous Subcutaneous Insulin Infusion Therapy and Continuous Glucose Monitoring in Adults.

The guidelines suggest continuous subcutaneous insulin infusion and continuous glucose monitoring have an important role in the treatment of diabetes.

They recommend or suggest the use of insulin pump therapy as the optimal treatment in the following instances if the patient and their healthcare team is willing and able of using the device:

  • Adults with T1 Diabetes not at A1c goal, OR at A1c goal but continue to experience severe hypoglycaemia or high glucose variability
  • Adults with T1 Diabetes who require insulin delivery flexibility or improved satisfaction
  • Adults with T2 Diabetes with good adherence to monitoring and have poor glycaemic control despite intensive insulin therapy, oral agents, other injectable therapy and lifestyle modifications

They recommend or suggest the use of Real-time continuous glucose monitoring in the following instances if the patient and their healthcare team is willing and able of using the device:

  • Adults with T1 Diabetes who have A1C levels above target or are well controlled and wish to use the device
  • Adults with T2 Diabetes are suggested to use intermittent CGM with A1c levels above 7%

The writers of the guidelines are an Endocrine Society-appointed Task Force of seven experts, a methodologist and a medical writer. The American Association for Clinical Chemistry, the American Association of Diabetes Educators, and the European Society of Endocrinology co-sponsored this guideline.

The task force developed the guidelines using the Grading of Recommendations,Assessment, Development, and Evaluation system to describe the strength of recommendations based on the quality of evidence.
